We are contracted with most insurance plans, and patients are urged to call their insurance company to verify this. Some of the major insurance plans we accept are:

Discounted Medical Care

We may be able to assist you with reducing the cost of your medical visit. We offer discounted services based on income and family size. To learn more, call our billing department to set up an appointment to complete an application. You will need to bring the following information to your application appointment:

1. Proof of income, for every household member. This may include:
   

   Three most recent paycheck stubs

   A copy of your current tax return

   Unemployment Information

   Social Security Income

   Child Support or Alimony

   Any other income

2. Proof of residence. For example, a bill addressed to you sent by mail.
3. A picture ID.
